This application is for undergraduate students who attended Alfred University in the past but did not complete their degree and are now seeking to come back and graduate. It must be completed by:

November 1 for students looking to return for the spring semester
July 1 for students looking to return for the fall semester.

Academic suspension (returning within the time frame of the suspension)

Academic suspension (did not return after the time frame of the suspension)

Academic dismissal (cannot be readmitted for two full years following dismissal from Alfred University)

Transferred to another college

Withdrew for personal reasons

Approved personal or medical leave (did not return after the approved time)

Approved personal leave (returning during the approved time)

Approved medical leave (returning during the approved time)

You do NOT need to complete this application form to return from an approved personal leave at the approved time. Please reach out to your assistant dean to complete any processes required in their office.
