Financial Responsibility Form

By receiving my course registrations for my first semester at Alfred University, I acknowledge and agree that I am financially responsible for all charges related to my registration.

• I understand that if I decide not to attend the courses, which I am being registered for, that it is my responsibility to drop the course(s) by the add/drop date to avoid any financial liability.
• I understand that if I utilize ancillary services or live on campus and incur charges such as housing, meal, course, and ancillary fees, these charges are also included as legitimate charges on my student account unless I notify the appropriate department and have the charges removed to avoid any financial liability.
• I understand that if financial payment has not been made by published due dates, a "hold" will be placed on my student record restricting me from registering until the account is paid in full.
• I understand that if my financial aid is reduced or cancelled, I am responsible for all charges on my account.
• I understand that if any portion of my account remains unpaid after a published due date, I will be responsible for any late payment fees assessed to my account.
• I authorize my school, their respective agents, and contractors to contact me regarding my debt, including repayment of my debt, at the current or any future number that I provide for my cellular telephone or other wireless device using automated dialing equipment or artificial or prerecorded voice or text messages.
• I understand, agree, and give express consent that Alfred University or anyone working on their behalf, including third party vendors, may contact me at the numbers provided to the University by manually dialing the number or by using automated dialing technology.
• I understand that if any portion of my account remains unpaid at the end of the semester, my account may be forwarded to a collection agency within 60-90 days.
• I understand that if the university does use a collection agency or take legal action for any account balance due, I will be liable for and shall pay all costs and expenses incurred by Alfred University, including reasonable attorney's fees and/or collection fees (which may be based on a percentage at a maximum of 33.3% of the debt) resulting from the referral.
